What you're buying

RxSpan MD is a physician-founded telehealth practice with four weight-loss products and three other lines beside them. On the GLP-1 side you can buy compounded semaglutide or compounded tirzepatide as a weekly injection, or either molecule as a capsule, and there is a microdosing option too. The rest of the menu is longevity — NAD+, sermorelin, glutathione — plus hair loss and sexual health, all prescribed through the same account.

The founder is named and credentialed: Dr. John Diaz, a plastic surgeon certified in his specialty with more than twenty years in practice, who says on the About section that treating cosmetic patients who kept asking about weight and nutrition is what led him to build this. The clinical group behind the prescriptions is Lion MD, led by Dr. Ana Lisa Carr and Dr. Kelly Tenbrink, both listed with NPI numbers you can check. Four dispensing pharmacies are named with phone numbers and websites: Belmar, Strive, Epiq Scripts and Casa Pharma Rx. That combination — a named founder, named prescribers with verifiable identifiers, and named pharmacies — is better than most.

One thing to know before you compare this with anything else here. The prescribing group is Lion MD — the site's own words are "a trusted team of nationwide medical professionals led by Dr. Ana Lisa Carr, MD (NPI: 1689841744) and Dr. Kelly Tenbrink, MD (NPI: 1346482684)" — and that is the same group, with the same two physicians and the same two NPI numbers, that Sunlight and Pallas Health name as well. The pharmacy lists overlap too: all four RxSpan names appear on Sunlight's list of six. Shared clinical infrastructure is common in this market and there is nothing improper about it, but it does change what a comparison means. Choosing between these three is not choosing between three independent clinical opinions — much of what happens after you pay is the same operation behind a different storefront.

What it costs (and how it's billed)

Start with what RxSpan MD does not publish, because it matters more than any figure it does. You pay before a clinician reads your intake — the plan card says so in its own words: "After checkout, a provider will review your information to determine if treatment is right for you. Prescription is not guaranteed." And there is no refund policy on the site. Not a page, not a paragraph, not a sentence: the word "refund" appears nowhere RxSpan MD publishes, and the terms of service, last updated in June, run to seven short sections and stop at Payments, which says only that you agree to pay all fees. The cheapest rate the site advertises is a single upfront payment of $2,094 for a year of semaglutide, and the same card adds "Auto-renews." A cancel-anytime bullet sits in the panel beside it, and nothing anywhere says what canceling gets you back.

Every plan bundles the same things: the consult, the medication, supplies, shipping and support, with no add-ons. What changes is how long you commit. When we read the plan selector in August 2026 it showed four rungs for compounded semaglutide: $249/mo billed monthly, $219/mo on a 3-month plan, $199/mo on a 6-month plan, and $175/mo on the 12-month plan with the sitewide STARTNOW code applied. The product card spells out what that last rung really is — "billed as one upfront payment of $2,094" — so the cheapest number on the page is a single charge of more than two thousand dollars.

Compounded tirzepatide injections are $329/mo month-to-month and fall to $243/mo on the 12-month plan, billed as one payment of $2,905 up front. The capsule versions are $249/mo for semaglutide and $349/mo for tirzepatide, with the same ladder behind them, and the semaglutide capsule has the lowest annual rung on the site at $162 a month, one payment of $1,933 up front. Every product is flat at every dose.

Brand access is real but priced as a first-month figure only: Zepbound $1,069, Wegovy $1,349 and Ozempic $1,299, each labeled "(For the first month)" with no renewal rate published anywhere. CareCredit financing is offered, subject to credit approval. Plans can be changed or canceled at any time, and the FAQ says there are no long-term commitments — which is true of the monthly rung and hard to square with a plan you pay for in one go.

Who can order

The FAQ says care is available in all fifty states, qualified with the usual line that availability of specific services may vary by state based on provider licensure and applicable regulations. No state-by-state list is published, and no product page names an exception.

Most members are approved on their written assessment alone; the FAQ says a brief telehealth visit is scheduled only if a clinician needs more information. There is no published minimum BMI and no insurance lane — this is cash pay.

How the intake works

Five steps, and the site is unusually plain about them. You complete a secure medical intake that takes about five minutes, you are matched with a provider, a licensed physician reviews and approves the plan, the prescription goes to one of the named pharmacies, and the provider stays with you for check-ins and refills. Unlimited appointments and messaging are included at every rung, and shipping is free and expedited.

A prescription is not guaranteed, and the site says so before checkout: a provider reviews your eligibility after you pay. What it never says is what happens to your money if that review goes against you — the gap described above, which several competitors here have closed with a single sentence.

What we'd want changed

Publish a refund policy. That is the whole of the first request. RxSpan takes payment before a clinician reads the intake, says plainly that approval is not guaranteed, sells a plan that costs $2,094 in one charge and auto-renews, and then leaves the one question that follows from all of that unanswered. A sentence would fix it.

The brand pricing is the second. Publishing $1,069 as a first-month figure and never printing the renewal is exactly the pattern this site avoids on its compounded line, and it undoes the goodwill the compounded ladder earns. And we would put the shared clinical stack on the page rather than leaving a shopper to find it: Lion MD, the same two prescribers and an overlapping pharmacy list also stand behind Sunlight and Pallas Health, which are reviewed here too.

Bottom line

RxSpan MD publishes more of what matters than most: a named founder, named prescribers with checkable NPIs, named pharmacies, a full plan ladder and an all-inclusive price. Buy the monthly rung at $249 or $329 while you find out whether the medication suits you, and treat the advertised low figures as what they are — one large payment made a year in advance, on a plan that renews itself. Get the refund terms in writing before you pay, because the site does not answer that question anywhere, and you are paying before anyone has decided you can have the medication.
