What a month costs across the field
Of the 508 providers tracked here, 157 file a tirzepatide price that is dated, unconditional and genuinely monthly. Those run from $126 to $599, with a median of $269. A quarter sit at or below $199 and a quarter at or above $330.
By band, none of the 157 come in under $100, 43 fall between $100 and $199, 69 between $200 and $299, and 45 sit at $300 or more. That right-hand weight is the whole difference from the semaglutide picture, where the same exercise puts 95 rows in the $100 to $199 band. Why the spread runs $126 to $599
The same four levers move a tirzepatide quote as a semaglutide one, and they move it harder because the base number is bigger.
Dose is the first: 105 of the 334 tirzepatide cells carrying a dollar figure are floors quoted at the starting strength, and tirzepatide titrates across six approved strengths, so the gap between the advertised month and the maintenance month is wide. Billing term is the second: 21 cells price a prepay plan, and a monthly figure that only exists because a quarterly charge was divided is not a monthly price. The Pallas Health review is the clearest case on this site of an advertised monthly average that is really one large charge split three ways.
What is bundled is the third lever. 6 cells are membership-gated, with a separate recurring fee beside the medication charge. Duration is the fourth: 9 cells are promotional rates and 3 are first-month rates. Only 174 of the 334 are a flat month-to-month figure at every dose. Where the law stands, as of August 2026
Compounded drugs are not FDA-approved; the agency does not verify their safety, effectiveness or quality before they are marketed2. That is true of every row above.
Tirzepatide's compounding lane narrowed first. The FDA declared the tirzepatide shortage resolved on December 19, 2024, and after a federal court declined to enjoin that decision in March 2025 the enforcement discretion for 503A pharmacies ended; the 503B window closed on March 19, 20251. The agency's current position is stated flatly on its own page: "Tirzepatide and semaglutide do not currently appear on the 503B bulks list or on FDA's drug shortage list"1. On April 30, 2026 the FDA proposed to exclude both, plus liraglutide, from that bulks list outright, "finding no clinical need for outsourcing facilities to compound these drugs from bulk substances"3. Comments closed July 30, 20264, and a Federal Register check on 2026-08-29 found no final determination.
Practically, that means a compounded tirzepatide vial today should come from a named, state-licensed 503A pharmacy compounding against your prescription. The brand comparison, and what you give up
Lilly's own savings page prices the approved product by strength: "Regular price for a 1-month supply of Zepbound single-patient-use KwikPen is $299 for 2.5 mg, $399 for 5 mg, $499 for 7.5 mg, $699 for 10 mg, $699 for 12.5 mg, and $699 for 15 mg"5. Against that ladder the $269 median above undercuts every strength above the lowest one, and it undercuts them by more as you titrate.
What the brand carries that no compounded vial does is its own evidence. In SURMOUNT-1, tirzepatide produced weight reductions up to roughly 21% at the 15 mg dose over 72 weeks6. That trial tested the manufacturer's product, not a compounded copy of it.
