What you're buying

Pomegranate Health is a general telehealth shop that happens to sell GLP-1s. The weight-loss aisle carries compounded semaglutide, Contrave and metformin, and the pricing page adds compounded tirzepatide and Rybelsus; the rest of the site is longevity injections, women's health creams, men's health compounds, hair and skin. If you want a service that thinks about nothing but GLP-1s, this is not it — though the breadth is genuinely useful if you are already buying two or three of those things separately.

The compounded formulations are named, which is more than most: semaglutide comes with cyanocobalamin, and tirzepatide with pyridoxine. So does the pharmacy list. The pricing page shows six compounders behind the products — BPI, Empower, Hallandale, RedRock, OptioRx and NOVA — and attaches a warning to one of them that USP 797 beyond-use-date limits let it dispense only a single vial per order. Very few sellers publish a constraint that inconvenient about their own supply chain.

What it costs (and how it's billed)

The pricing page moved from behind the app login to the open web, which is a real improvement, and in August 2026 it showed compounded tirzepatide at "$149/month" and compounded semaglutide at "$129/month," both on the Starter tier. Metformin, Contrave and Rybelsus are each listed as "As low as $0 copay," which is an insurance figure rather than a cash cost. Shipping is included, there is no membership and the site repeats "no consultation fee" throughout.

The trouble is that the product page has not caught up. On the same day, /products/compounded-semaglutide still read "Starts @ $119/mo" — ten dollars a month below the pricing page for the same product. We cannot tell you which one you will be charged, and neither can the site. If you are shopping on price, the number to plan around is the higher one.

The tier structure adds a second wrinkle. Each product offers Starter and Advanced, and on the Starter tier tirzepatide reads "$149/month." Select Advanced and the same $149 figure re-labels itself as "$149/2 vials," with 40 mg supplies offered in two-vial and three-vial packs. A number whose unit changes when you change tier is not a monthly price in any useful sense, and nothing on the page explains how many weeks two vials are meant to last. Alaska, Puerto Rico and Hawaii carry a $60 shipping surcharge, charged on every order, with delivery in three to five business days.

Who can order

There is still no served-states list, but the pricing page now publishes exclusions per product, which is the more useful half: semaglutide is marked "Not available in: CA, IA, WI" and tirzepatide "Not available in: HI." California and Michigan patients should read that line carefully before starting, because the exclusions attach to the pharmacy rather than to the company.

Eligibility is stated in a footnote on the same page and is stricter than the marketing suggests: patients new to a GLP-1 need a body mass index of 27 or higher with a related condition, or 30 or higher without one. Patients already on a GLP-1 with a lower index are assessed for maintenance dosing instead. Age is not stated anywhere we could find.

How the intake works

You answer a health questionnaire, a licensed physician reviews your history and decides whether you are a candidate, and if you are prescribed, the medication ships to your door at no extra cost. The site says delivery lands within days and offers ongoing access to a care team; the pricing page ends in a "Start Intake" button rather than a checkout, so the price you have just read is what the intake is priced against.

Support is the most concretely documented part of the site: Monday to Friday from 7 AM to 10 PM Eastern, Saturday and Sunday from 8 AM to 5 PM, by phone at 813-419-1189 or at support@joinpomegranate.com. Weekend coverage from a compounded seller is unusual and worth something. Klarna, Afterpay and Affirm are offered at checkout, so any monthly figure you see inside a financing widget is an installment and not the medication price.

What we'd want changed

Make the two pages agree. "Starts @ $119/mo" on the product page and "$129/month" on the pricing page for the same compounded semaglutide is the sort of gap that turns a good-faith shopper into a suspicious one, and it costs Pomegranate nothing to fix. The same goes for the Advanced tier's "$149/2 vials": if the Advanced product is priced per vial rather than per month, say how many weeks a vial covers.

Publish a states list rather than per-pharmacy exclusions alone. Knowing that semaglutide cannot ship to three states is useful; knowing which states can be prescribed in at all, and which require a live visit first, is what a shopper actually needs before spending twenty minutes on an intake.

Bottom line

Pomegranate has quietly become one of the more readable GLP-1 sites: six named pharmacies, named co-formulations, published per-product state exclusions, a real eligibility rule and weekend support hours, all without a membership or a consultation charge. Plan on $129 a month for semaglutide and $149 a month for tirzepatide on the Starter tier, treat the product page's $119 a month as stale rather than as a deal, and read the Advanced tier's unit before you buy it.
